CLABSI reduction efforts include house-wide, multidisciplinary participation, a continuous process improvement framework, and leadership support. Ongoing areas of opportunity include maintaining engagement, standardizing CVC rounds discussions, and increasing family engagement.

BCH Code White Committee

 

 

 

 

The BCH Code White and Rapid Response subcommittee is a UCSF Medical Center Committee responsible for the review and analysis of code white emergency response and outcomes in BCH. The committee also provides oversight of the BCH Rapid Response Team and is responsible for monitoring and evaluating RRT outcomes and guiding process improvements. (Also reports to the Code Blue Committee)

Group comprised staff RN unit reps, PI team member, Physical Therapy staff rep, and Safe Mobilization team rep.  We review monthly unit data, including fall events, audits, and IRs.  Additionally, we identify gaps and address them as needed.  The group is also involved development of new tools, educational needs.

Trained Family advisors who partner with staff and clinicians to provide advice on various topics from policy changes to designing a healing environment to clinical care from a parent perspective. Families represent various units, diagnoses and types of admissions. The FAC mission is to optimize the quality of patient and family experiences at UCSF Benioff Children’s Hospital by incorporating the principles of Family Centered Care.”

BCH Patient Safety

The Patient Safety Committee is a multidisciplinary medical staff committee. The Committee: Provides consistent oversight of the investigation and mitigation of adverse events, conducts root cause analysis activity, monitors the effectiveness of risk reduction strategies and measures of change to ensure ongoing system improvement, reviews and evaluates incidents ranked as serious, assigns responsibility for follow-up, identifies systems issues that would lead to the likelihood of the event occurring in other settings and delegates’ appropriate actions to mitigate those future risks, reviews aggregate adverse event and incident report data for issues and trends, prioritizes safety system issues that require action, develops implements and monitors the Patient Safety Plan (Policy 1.02.17), each year, selects at least one high-risk process and assures that a proactive risk assessment (FMEA) is conducted and presented to the committee, provides oversight of the analysis and response to Joint Commission Sentinel Event Alerts, provides oversight and evaluates performance data and issues related to the National Patient Safety Goals (k) Determines system wide measures and related benchmarks of patient safety, establishes performance targets and advises leadership of variability (l) Communicates patient safety issues to QIEC to foster integration of efforts among the quality committees.
